SUBJECT: Survey of Bank Officer Loans and Overdrafts

In reviewing the forms returned to date, it has been found that many banks have misinter-
preted our request for information on Schedules II and III of the survey. Schedule II deals
with loans made to officials and major stockholders of other banks, while Schedule III per-
tains to loans made to officials, major stockholders and other personal and business interests
of the reporting bank. Please note, as the instructions specify, to report only those loans
outstanding as of September 30, 1977 to the indicated borrowers, EXCEPT for the
exclusions listed in the instructions for completing Schedules II and III. If you have not yet
submitted the survey, please review Schedules II and III with regard to these reporting re-
quirements. If you have already submitted the survey forms, any amendments to the infor-
mation already reported should be clearly indicated as such.

If you need additional forms for making any corrections necessary, assistance or other infor-
mation regarding this survey, the following toll-free number is available for banks in the

not supplied certain required information. Specifically, under Schedule IV (Overdrafts),
Item "e" of parts 1, 2 and 3, and all of parts 4 and 5, MUST BE COMPLETED BY ALL
INSURED COMMERCIAL BANKS. Please review your survey materials to insure that this
information has been furnished.

If you have already submitted the survey with the above required items, no further action
on your part is necessary at this time. If your forms have been submitted without the
required information, please supply the OMITTED ITEMS ONLY on the other side of this
letter and return to the FDIC in the envelope enclosed within three days. All survey forms
should be received at the FDIC by December 9, 1977.

If you have not received the questionnaires sent to you on October 31, 1977, please call us
on the toll-free numbers for a set of questionnaires and instructions.
